How It Works
Our sliding scale fee program adjusts the cost of medical services based on your household income and family size, following Federal Poverty Level (FPL) guidelines and making your visits more affordable.
100% FPL or Below
75% Discount
101-200% FPL
50% Discount
201-400% FPL
25% Discount

Eligibility Requirements
- Must be uninsured or underinsured
- Household income at or below 400% of Federal Poverty Level
- Must provide documentation of income and family size
- Re-certification required annually
Required Documentation
- 2 months of recent pay stubs or recent tax returns
- 2 months of unemployment or disability benefit statements
- Proof of family size and composition
- Documentation of any other income sources
