First Visit Information
Everything you need to know to prepare for your first appointment with us.

Call us at 888-795-1110 or use our patient portal to book your first visit. You will receive an appointment reminder via call, text, or email prior to your visit.
To cancel or reschedule, please call us at least a day before, or at least one hour prior to your scheduled time.
Complete Paperwork
Fill out new patient forms including medical history, insurance information, and consent forms.
Arrive Early
15 minutes early for follow-up appointments for smooth check-in. 30 minutes early for new patient appointments — this allows time for check-in and paperwork.
If you arrive more than 15 minutes late, your appointment may need to be rescheduled. After three missed appointments, you may need to walk in and wait to be squeezed in, but we cannot guarantee same-day availability.
Bring Required Items
Bring your insurance card if you have one, valid photo ID, list of current medications, relevant medical records and payment method.
What to Expect During Your Visit
- Check-in and insurance verification (15 minutes)
- Information on additional costs such as applicable copays, deductibles, or coinsurance.
- Comprehensive health assessment and medical history review
- Physical examination and vital signs
- Discussion of health concerns and treatment options
- Care plan development and next steps
- Scheduling follow-up appointments if needed
